2011 - 2012 Protection Policy Forms

Required Background Check on all Volunteers
In order to provide our children with the best security we can, we require all volunteers to complete a set of volunteer forms, including consent to a background check.  (If you have volunteered in the Children's Ministry at Christ Church of Oak Brook in the recent past, you might not need to complete all the paperwork.  If you call Linda Barnett in the Children's Ministry Office 630.321.3928, she will confirm what paperwork you need to complete.)

Please print each form listed below, complete each one, and return to Children's Ministry Office at Christ Church of Oak Brook, 501 Oak Brook Rd, Oak Brook, IL  60523.

•  Volunteer Application form (only one complete sentence is necessary for each question, and there is no grading on content!)
•  Annual Renewal Volunteer Application
•  Background Consent Form
•  Protection Policy Form

We truly appreciate all the time required of you to complete this paperwork, which is only necessary for the parent who is volunteering.  For your privacy, please know that all completed forms are kept in a private file of which only a select few leadership staff at Christ Church of Oak Brook have access.  Please fill out your forms now while it is fresh in your mind.
